Though the controversial album art depicts a menacing shadow of a man approaching a young girl on a swing, Ferrara says "he was a super-nice guy; he was doing me a favor, by blocking my eyes from the sunlight." The image was later digitally edited to look more threatening.

Once Korn became popular, Ferrara’s parents worried that she would be recognized by fans, and that they may get in trouble for using her real private-school outfit in the photo.
